Choose a protocol suited to the patient, the patient's work-up and context, including the cost of treatment.
A protocol is not picked from a catalogue: it is deduced from the work-up. Ovarian reserve, age, history, body habitus, response to previous cycles — you learn to connect these to a precise decision: agonist or antagonist, starting dose, adjustments along the way, choice and timing of the trigger.
The cost of treatment is part of the reasoning, not a footnote. A prescription the patient cannot see through to the end does not produce a half-successful cycle: it produces an interrupted one. You also work on situations where the number of possible attempts is limited, and where each cycle has to be decided accordingly.
Sessions are held live on Zoom, on real anonymised records of African patients: the initial work-up, the protocol chosen, the doses actually prescribed, and what the cycle led to.
